I don't think that there is any question that the rules should be disregarded because a particular infringement was made by mistake, or by an inexperienced player.
The rules are not in question, it is the application of penalties for infringement that needs common sense and a reasonable approach by supervisors.
A supervisor has a wide range of penalties available to him ranging from a warning to outright disqualification, and in some cases taking no action at all.
A player who calls what he thinks is the blind without realising that there had been an all in in front of him, need not be forced to put all his chips in if in the opinion of the supervisor the original raiser has not been disadvantaged in any way, and the there was no apparent angle shoot by the player, and as spoona has already said the action stopped at the erroneous player.
Every case is different, but outcomes should be fair to both the players and the game as a whole, and for this we depend on having strong knowledgeable cardroom supervisors who the players have confidence in.
Luckily most of the people who run our local rooms fall into this catergory.
